Job Summary: Create a plugin for Wordpress that will allow the admin to create multiple unique contact forms that can be placed in the upper right hand sidebar. The data collected from the contact form will need to be posted to a database so that the admin can track information and query the database for results. We use and like the plugin Contact Form 7 – you could start with this and enhance it or you could create a brand new plugin. Job needs to be completed as soon as possible, no more than 5 days. Details: See attached powerpoint for a visual representation of contact form. Keep in mind, the actual design and colors used do not represent exactly what we want, this is only to help you visualize the contact from process. The form design should fit in with the fonts and colors used on the site. Create one contact form template, with the data fields shown in the powerpoint and split the data fields into multiple steps. Step 1 = data field requested in sidebar (first name, last name, email, verify email) and step 2 = data fields requested in next screen of contact form (address, city, state, zip, phone, due date, comments/questions). The form that pops up for step 2, should be ajax so that page does not have to refresh or reload. Forms must use akismet and be 100% spam proof – no captcha Admin must be able to: -Create new contact forms (multiple) -Add/Delete data fields from the template in order to create unique and different contact forms -Add/remove steps – be able to create a 3rd step or 3rd screen of the contact form in order to gather additional information -Control what page/post hosts which contact form -Control where the data of the form is sent to (multiple email addresses) Data from form must be posted to database so that we store all information. Must capture data after each step so that if step 2 is not completed then we still capture data from step 1. We want to keep this very simple – a basic table with input fields for the start and end dates should be enough (see attached spreadsheet for visual representation).
